An organisation is a machine for turning arrivals into committed effects. Orders arrive and filled orders leave; claims arrive and settlements leave; a promise is made and either kept or broken. Around that machine sits a second one, made of meetings, reports, handoffs, approvals and screens. The second machine is not the work. It is what the work costs when it is run on minds that cannot see the same state at the same time. Every layer of infrastructure beneath it — silicon, storage, network, database, application — has been engineered ruthlessly for sixty years, and every one of them terminates at a screen, where it stops and waits for a skull. The cognition layer itself was never engineered, because it could not be: it ran inside people, and you cannot optimise what you cannot virtualise. It can be now. And the moment it can, the second machine does not become efficient. It becomes undefined, in the way that a monitor's contents become undefined when nothing is looking at it.
Concretely, and in the vocabulary of the people who will have to build it. One append-only, hash-chained tape whose typed alphabet is the firm's own schema, so that every table is a fold over the record and the record is simultaneously the token stream. A resident trunk that never returns: no turn boundary, a key-value cache that is never discarded, end-of-thought read off logits the ingest decode already produced at zero marginal cost, and judgments taken on zero-copy forks of the live state rather than on fresh prompts. Per-decision heads placed after the trunk and never inside it, emitting typed choices over closed alphabets with calibrated margins instead of decoding prose that another program must then parse. A stencil relaxation over the commitment lattice that prices contention, scarcity and deadline pressure across every open promise before a single token is spent on any of them. A deterministic gate, compiled as its own library with its hash on every decision it authors, that reads the proposal's signed direction, its sharpness against the class's own spread, a conformal novelty score computed from the record rather than from the model's self-report, the licence for that class, the reversibility derived from the adapter rather than declared by whatever wants to act, the budget and the outstanding exposure — and emits exactly one verb with a reason from a closed vocabulary. A single writer that records the inverse before the effect leaves. And an anytime-valid evidence process over exterior verdicts on the machine's own executed choices, drawn on a sample it cannot predict, which widens authority only inside a rule a person signed once and narrows it on evidence alone. Nothing learned ever disposes.
Take a concrete case, and take the hardest kind: an allocation problem that is life-critical, time-critical, and already instrumented by a regulator. A deceased donor; a ranked match run; on the order of two hundred and fifteen offer notifications per donor against a median run of about fifteen; and a window measured in hours before the organ is no longer viable. The early offers are handled. The fourth and fifth organs are lost in the tail, an order of magnitude beyond the median, where nobody had bandwidth for the seventieth call. Inside the organisation that runs it, the frontline recovery role carries 1.30 people above each person on the wire while the placement role, in the same building, in the same year, on the same budget, carries 0.00; there are 128 distinct job titles for 402 people, 62 percent of them held by exactly one person; and a headquarters built in 2022 has never been half full, which is an experiment the organisation ran on itself and declined to read. None of that is a story about bad management. It is what an interior does when nothing is eating it — visible here only because a federal protection removed the thing that normally erodes one.
